Very few people begin tirzepatide therapy with any thought for their teeth, since the early conversation concerns nausea, dose escalation, and whether the scale is going to move. Several months later a routine cleaning may turn up two small cavities in someone who had not had one since their twenties.
The mechanisms behind that are ordinary. Reduced fluid intake, a drier mouth, occasional exposure to stomach acid, and a much smaller volume of food all converge on one enclosed environment that depends on a narrow set of chemical conditions to remain stable.
Dry mouth is reported during tirzepatide therapy, although it did not rank among the most frequently recorded adverse events in the registration trials, where gastrointestinal effects predominated. In the SURMOUNT-1 program nausea affected roughly a quarter to a third of participants depending on dose, so the picture for xerostomia comes as much from patient reports as from trial tables.
The more useful question concerns why it happens. Tirzepatide reduces appetite, and for most people the drive to eat and the habit of drinking travel together, so the water consumed alongside meals tends to disappear with them, while vomiting and diarrhea remove fluid directly. There is no strong evidence that the medication acts on the salivary glands the way anticholinergic drugs do, so an explanation resting on fluid balance is adequate.

Saliva performs several jobs that keep enamel intact, and losing volume compromises all of them at once. The first is buffering, since saliva carries bicarbonate and phosphate systems that neutralize acid arriving from bacteria or from the stomach, and enamel demineralizes below a pH of roughly 5.5.
The second job is remineralization, because saliva is supersaturated with calcium and phosphate that deposit back into enamel which has partially dissolved. The third is mechanical and antimicrobial, since saliva clears food particles and holds the oral microbial population in a balance that shifts toward acid producing species once flow declines. Hyposalivation, defined by an unstimulated flow rate below roughly 0.1 mL per minute, is associated with measurably higher rates of dental caries.

Stomach acid sits at a pH of roughly 1.5 to 3.5, far below the level at which enamel dissolves, so while a single episode does little harm, repeated episodes across months produce a recognizable erosion pattern on the upper front teeth.
Two situations connected to treatment put acid in the mouth. The first is vomiting, reported in roughly 8 to 12 percent of participants across doses in SURMOUNT-1 against under 2 percent for placebo. The second is reflux, described commonly during GLP-1 therapy because slowed gastric emptying keeps stomach contents in place longer, and silent nighttime reflux matters most because salivary flow falls during sleep.
The most important practical point runs against intuition, since acid softens the enamel surface temporarily and a toothbrush then abrades material that would otherwise have remineralized. Dental guidance is to rinse with plain water, or with baking soda dissolved in water, and to wait about 30 minutes before brushing.

It can, because decay risk tracks the frequency of acid exposure far more closely than it tracks total food volume, which surprises people who assume that cutting intake by a third should have lowered their risk.
Hard candies and ginger chews used for nausea are often sucked slowly and repeatedly through the day, which produces close to the worst possible exposure profile for enamel, while sweetened electrolyte drinks deliver both sugar and citric acid and are usually sipped rather than finished. Substantially reduced intake can also compromise micronutrients supporting the gums, including vitamin C and protein itself.

The useful interventions are ordinary ones, and none of them requires abandoning treatment. Sipping water steadily through the day addresses dryness more effectively than drinking a large volume at once, because salivary production responds to hydration status across hours rather than minutes.
Stimulating saliva deliberately is the second lever, since sugar free gum sweetened with xylitol increases flow mechanically and carries reasonable published support for reducing caries risk. Fluoride toothpaste used twice daily is the baseline, and spitting without rinsing leaves a higher residual concentration on the teeth, while dentists can prescribe a 5,000 parts per million formulation wherever caries risk has demonstrably risen.

Most comfort measures used during escalation can be kept, since the substitutions that protect enamel are close variants of what people already reach for.

Comparisons reflect general dental guidance on erosive and cariogenic exposures. Individual circumstances differ.
A reasonable default is a dental check roughly six months into therapy, brought forward wherever vomiting recurs around escalations, nighttime reflux becomes regular, or dryness persists. New sensitivity to cold or sweet deserves attention rather than tolerance, since it is often the first sign that enamel has thinned enough to expose the dentin underneath.
Tell them the medication name and the dose, and mention whether vomiting or reflux has been part of the experience, because that history changes how a dentist reads new erosion and allows preventive fluoride to be considered.
Sedation is the second reason and it carries more weight. Delayed gastric emptying during GLP-1 receptor agonist therapy prompted preoperative guidance from the American Society of Anesthesiologists in 2023 on whether to hold weekly injectable agents before elective procedures requiring sedation. Any decision about holding a dose belongs with the prescribing clinician and the sedation provider together.
Tirzepatide changes the environment inside the mouth through unremarkable mechanisms, since less fluid comes in, acid occasionally comes up, and consumption shifts toward smaller and more frequent exposures. All of it responds to preventive measures dentists have recommended for decades.
The failure mode worth avoiding is silence, because dental change during GLP-1 therapy tends to be gradual and painless until it is neither.
