Tirzepatide does not carry a long list of absolute drug contraindications, and that fact tends to end the conversation earlier than it should. A prescriber checks for a family history of medullary thyroid carcinoma, asks about pancreatitis, and the rest of the medication list often receives a quick scan rather than a genuine review.
The interactions worth understanding differ from the textbook version where one drug blocks the enzyme that clears another, because tirzepatide slows the rate at which the stomach empties while also producing substantial weight loss. Neither of those appears as a red flag in an interaction checker.
Tirzepatide has few interactions in the traditional metabolic sense, though it has meaningful ones in absorption and dose appropriateness, because it is a peptide cleared through proteolytic breakdown rather than through the cytochrome P450 enzyme system where most classic drug interactions originate. That is why an interaction checker returns such a short list.
What the checker does not model is the mechanical effect, since a tablet swallowed with breakfast may sit in the stomach considerably longer before reaching the small intestine where most absorption happens. Tirzepatide alone carries a low risk of hypoglycemia because its effect on insulin release is glucose dependent, whereas glipizide, glyburide, and injected insulin do not work that way.
Stacking a glucose lowering agent onto a fixed insulin dose while food intake is falling creates the conditions for hypoglycemia, and the risk tends to be highest during the first weeks and after each escalation. Metformin and SGLT2 inhibitors are commonly continued alongside tirzepatide, while DPP-4 inhibitors act on the same incretin pathway and are viewed as redundant rather than dangerous.
Delayed gastric emptying changes when an oral medication is absorbed and, for some drugs, how much of it reaches the bloodstream, which is why the prescribing information advises caution with oral medications that have a narrow therapeutic index. Warfarin is the standard example, alongside levothyroxine, certain antiepileptic drugs, digoxin, and some immunosuppressants.
Two details make this more manageable than it sounds, since the effect is most pronounced after the first dose and diminishes with continued exposure, which concentrates the concern into identifiable windows rather than years of treatment. The change also alters the timing of the peak rather than total exposure, which matters far more for a drug taken for an acute effect than for one maintaining a steady state.

Tirzepatide reduces the absorption of oral hormonal contraceptives, and the prescribing information advises patients using them to switch to a non oral method, or to add a barrier method, for four weeks after starting tirzepatide and for four weeks after each dose increase.
The pharmacokinetic study behind that recommendation showed a substantial fall in the peak concentrations of both the estrogen and the progestin components, with a much smaller reduction in total exposure. This item is among the most consequential on the list and the most frequently missed, particularly for anyone who started therapy for weight management and did not think of the visit as a contraception conversation.

Blood pressure typically falls during tirzepatide therapy, so an antihypertensive regimen set at a heavier weight can gradually become too aggressive. Published results from SURMOUNT-1 reported systolic blood pressure decreasing by approximately 7 to 8 mmHg at higher doses, and reductions of that size are clinically meaningful.
The symptoms that follow are easy to attribute to the wrong cause, since lightheadedness on standing and afternoon fatigue tend to be filed under side effects of the GLP-1 medication when the explanation may be a blood pressure now running too low. Diuretics deserve particular attention, because combining one with an ACE inhibitor or an ARB and an NSAID during an episode of vomiting is a recognized setup for acute kidney injury.

Several widely prescribed medications are associated with weight gain, and they matter because the interaction runs in the opposite direction from the ones above, with the other drug working against the purpose of the therapy. The classes include certain second generation antipsychotics such as olanzapine and quetiapine, some antidepressants including mirtazapine and paroxetine, gabapentinoids, systemic corticosteroids, and some beta blockers.
None of this means those medications are wrong for the person taking them, since many treat conditions where the alternative is worse than a few kilograms, and stopping a psychiatric medication to improve a weight trajectory carries real consequences. The question still deserves to be asked, because an alternative within the same class sometimes carries a more neutral weight profile.

A complete list is the entire exercise, and complete means considerably more than the prescriptions, since over the counter products, supplements, and anything taken occasionally all belong on it alongside the dose and the time of day each one is taken.
Timing matters more than people expect, because several of the practical adjustments turn out to be scheduling changes rather than substitutions, and moving a medication away from the day of the weekly injection can resolve a problem without altering anything prescribed. It is also worth naming the monitoring plan explicitly, meaning which labs will be checked and at what interval.
The interaction profile of tirzepatide rests on two mechanisms rather than a long list of chemical conflicts, since gastric emptying slows in a way that matters most in the early weeks, while weight and glycemic control improve in a way that makes several existing doses too high.
