In the Zepbound vs Wegovy comparison, the one randomized head to head trial in adults with obesity found greater average weight reduction with Zepbound's active ingredient, at 20.2% of body weight compared with 13.7% for Wegovy's over 72 weeks. Weight loss is only part of the decision, because the brands differ in approved uses, dosing, formats and self pay prices.
Zepbound is Eli Lilly's brand of tirzepatide for weight management and obstructive sleep apnea, and Wegovy is Novo Nordisk's brand of semaglutide for weight management, cardiovascular risk reduction and a form of fatty liver disease. The molecules and both head to head trials are covered in more depth in our tirzepatide vs semaglutide trial review.
Key Takeaways
- In SURMOUNT-5, an open label trial of 751 adults with obesity and without type 2 diabetes, average weight change at 72 weeks was 20.2% with tirzepatide (Zepbound's ingredient) and 13.7% with semaglutide (Wegovy's ingredient).
- The Zepbound label covers weight reduction and moderate to severe obstructive sleep apnea in adults with obesity; the Wegovy label covers weight reduction, reduction of major cardiovascular events in adults with established cardiovascular disease, and noncirrhotic MASH with moderate to advanced fibrosis.
- Zepbound starts at 2.5 mg weekly and rises in 2.5 mg steps after at least 4 weeks per dose to 5 mg, 10 mg or 15 mg; Wegovy injection starts at 0.25 mg and reaches maintenance from week 17, usually at 2.4 mg, with 7.2 mg available for some patients.
- Self pay prices run from $299 to $449 per month for the Zepbound multidose pen and $349 to $399 for the Wegovy injection, with the Wegovy pill at $149 to $299.

How does Zepbound work compared with Wegovy?
Zepbound works by activating two gut hormone receptors, GIP and GLP-1, while Wegovy activates the GLP-1 receptor alone. Both slow stomach emptying and act on appetite pathways in the brain, and the added GIP activity is the leading explanation for tirzepatide's larger average effect, although each receptor's contribution in humans is still being studied. Our explainer on how tirzepatide's dual GIP and GLP-1 action works covers the mechanism in detail.
Both labels carry a boxed warning about thyroid C-cell tumors seen in rodents, and both advise against combining the medicine with another GLP-1 receptor agonist, so the two are alternatives rather than partners.
Is Zepbound better than Wegovy for weight loss?
Zepbound produced more average weight loss than Wegovy's active ingredient in the only direct comparison, so on that single measure it has the stronger evidence for adults with obesity and without diabetes. SURMOUNT-5, published in the New England Journal of Medicine in 2025, randomized 751 adults to the maximum tolerated dose of tirzepatide (10 mg or 15 mg) or semaglutide (1.7 mg or 2.4 mg) once weekly for 72 weeks. Weight fell by an average of 20.2% with tirzepatide and 13.7% with semaglutide, a gap of 6.5 percentage points, and waist circumference fell by 18.4 cm and 13.0 cm respectively.
The trial was open label and funded by Eli Lilly, the maker of Zepbound, and it did not test the 7.2 mg Wegovy dose the label now allows. Individual response within each group also varied widely.

The separate placebo controlled trials point the same way, with the caution that results from different trials cannot be compared directly. In SURMOUNT-1, tirzepatide produced average weight changes of 15.0%, 19.5% and 20.9% at 5 mg, 10 mg and 15 mg over 72 weeks, against 3.1% with placebo, while in STEP 1 semaglutide 2.4 mg produced an average change of 14.9% at 68 weeks, against 2.4% with placebo.

Which conditions is each medicine approved for?
Zepbound and Wegovy share one indication, chronic weight management, and each holds approvals the other does not. According to the Zepbound prescribing information, Zepbound is approved alongside diet and physical activity to reduce and maintain body weight in adults with obesity, or with overweight and at least one weight related condition, and to treat moderate to severe obstructive sleep apnea in adults with obesity. The Wegovy prescribing information lists weight reduction in adults and in adolescents aged 12 and older with obesity, reduction of major adverse cardiovascular events in adults with established cardiovascular disease and obesity or overweight, and, under accelerated approval, noncirrhotic MASH with moderate to advanced liver fibrosis (stages F2 to F3).
Each extra indication rests on its own trial. SELECT enrolled 17,604 adults with cardiovascular disease and overweight or obesity but no diabetes, and over a mean follow up of 39.8 months a major cardiovascular event occurred in 6.5% of the semaglutide 2.4 mg group compared with 8.0% of the placebo group, a hazard ratio of 0.80. The SURMOUNT-OSA trials enrolled adults with moderate to severe sleep apnea and obesity, and our article on Zepbound's sleep apnea approval walks through what they found.

The better fit can therefore depend on a person's other diagnoses, since established heart disease points to Wegovy's label and sleep apnea to Zepbound's, and insurers often tie coverage to these labeled uses.
How do the Zepbound and Wegovy dosing schedules differ?
Both medicines start low and rise gradually to limit gastrointestinal effects. Zepbound begins at 2.5 mg once weekly for 4 weeks and then moves to 5 mg, after which the dose may rise in 2.5 mg steps after at least 4 weeks at each level, with maintenance doses of 5 mg, 10 mg or 15 mg for weight management, 10 mg or 15 mg for sleep apnea, and 15 mg as the maximum. Wegovy injection follows a fixed table of 0.25 mg for weeks 1 through 4, 0.5 mg for weeks 5 through 8, 1 mg for weeks 9 through 12 and 1.7 mg for weeks 13 through 16, with maintenance from week 17 at 2.4 mg or 1.7 mg; after at least 4 weeks on 2.4 mg, the dose may rise to a maximum of 7.2 mg.

Dose Escalation: Zepbound vs Wegovy Injection
Source: Zepbound and Wegovy prescribing information, DailyMed (revised 8/2026 and 6/2026). Zepbound shows the earliest possible schedule to 15 mg; Wegovy 7.2 mg not shown. Doses are not equivalent across drugs.
The Wegovy tablet adds a format Zepbound does not have. It starts at 1.5 mg once daily for 30 days, steps up every 30 days through 4 mg and 9 mg, and settles at 25 mg once daily, taken on an empty stomach in the morning with no more than 4 ounces of water and followed by a wait of at least 30 minutes before food, drink or other oral medicines. Zepbound is injection only and is supplied as single dose pens, single dose vials, multi dose vials and a single patient KwikPen holding 4 doses.
Zepbound vs Wegovy cost: what does each cost without insurance?
Without insurance, both medicines cost a few hundred dollars per month through each manufacturer's self pay program. Lilly sells the multidose Zepbound KwikPen to cash pay patients through LillyDirect, where the lowest dose is priced at $299 per month, and the company has said LillyDirect patients will pay no more than $449 a month for the multidose pen, as TechTarget reported. Lilly attaches refill timing conditions to its higher dose prices, so current figures should be confirmed directly.
NovoCare, Novo Nordisk's pharmacy, lists the Wegovy injection at $349 per month for doses from 0.25 mg to 2.4 mg and $399 for Wegovy HD 7.2 mg, with $199 for each of a new patient's first 2 monthly fills of 0.25 mg and 0.5 mg, an offer listed through December 31, 2026. The Wegovy pill is listed at $149 per month for 1.5 mg, $199 for 4 mg and $299 for the 9 mg and 25 mg doses, against a list price of $1,349.02 per package.

Insurance coverage follows separate rules based on the plan, the labeled indication and prior authorization, which our article on how plans decide GLP-1 coverage explains.
How do the side effects of Zepbound and Wegovy compare?
The side effect profiles of Zepbound and Wegovy are broadly similar and are dominated by gastrointestinal effects. In SURMOUNT-5, the most common adverse events in both groups were gastrointestinal, mostly mild to moderate, and occurred mainly during dose escalation. In STEP 1, 4.5% of semaglutide participants stopped treatment because of gastrointestinal events, compared with 0.8% on placebo. Both labels say neither medicine should be used by people with a personal or family history of medullary thyroid carcinoma or MEN 2, and both list the same serious warnings: pancreatitis, gallbladder disease, low blood sugar when combined with insulin or a sulfonylurea, kidney injury from dehydration, and allergic reactions. Severe abdominal pain that does not go away needs prompt medical care. Neither medicine is meant for use during pregnancy: the Zepbound label says to stop when a pregnancy is recognized, and the Wegovy label says to stop at least 2 months before a planned pregnancy. The Zepbound label also advises people on oral hormonal birth control to switch to a method that is not taken by mouth, or add a barrier method, for 4 weeks after starting and for 4 weeks after each dose increase.
Zepbound vs Wegovy vs Ozempic: where does Ozempic fit?
Ozempic contains semaglutide, like Wegovy, but is a separate product approved for type 2 diabetes, and Mounjaro has the same relationship to Zepbound as tirzepatide labeled for type 2 diabetes.
